Overview:

MDS (Minimun Data Set) Coordinator 

  • Schedule : 8am to 5pm Full Time-Day Shift 
  • Department : Skilled Nursing Unit 
  • Location : Presbyterian Medical Center – Charlotte, NC
  • Responsibilities   Facilitates the timely and accurate completion of the minimum data set; certifies the need for appropriate care with awareness of costs, length of stay, and determines when level of care changes; insures successful transmission of data to the State. No on-call, no floating to the floor. 
  • 12 Bed Unit, Medicare Only

Qualifications:
  • Education: High School Diploma or GED, required. 4 Year / B achelors Degree, required. BSN, required.
  • Experience: 3 years of current MDS experience within the last 12-18 months, required.
  • Licensure/certification/registration: Currently licensed as RN in appropriate state, required. AANAC Certification, preferred.
  • Additional skills required: Effective interpersonal relationship skills; commitment to continually improving skills through participation in educational opportunities whether offered as on-the-job, within, or outside the health system to increase knowledge of work-related issues; recognizes, respects, and successfully works with diversities among staff and others; effectively serves as a staff educator regarding MDS and Medicare; ability to see the big picture and act as a systems thinker; demonstrates effective organizational skills.

The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.

If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
